ACCESS Plus, Pennsylvania’s enhanced primary care case management program, is one of 15 finalists for the 2008 Innovations in American Government Awards, as selected by The Ash Institute for Democratic Governance and Innovation at Harvard’s Kennedy School (John F. Kennedy School of Government).

Made possible through a partnership with McKesson Health Solutions, a business unit of McKesson Corp, ACCESS Plus delivers health care services to low-income children and families.

The program has helped participants better manage chronic illnesses such as diabetes, heart problems, and asthma since March 1, 2005.
